One of the first symptoms of low testosterone to improve with testosterone replacement therapy (TRT) is fatigue. However, there are times when fatigue is still present even after six weeks of treatment. There are three notorious contributors to fatigue that may not be alleviated by testosterone replacement therapy: adrenal function, thyroid function, and sleep apnea.
The adrenal glands sit on top of your kidneys and control important bodily functions, namely adrenaline and stress. Adrenal fatigue is a serious condition because your body is unable to release the right hormones at the right time to respond to certain situations. Adrenal fatigue can lead to decreased libido, infertility, low blood sugar, low blood pressure, headaches, dizziness, anxiety, panic attacks, and yes, fatigue.
The thyroid is a butterfly-shaped endocrine gland at the base of the neck above the chest. The thyroid is designed to create T4 (thyroxine) that is chemically converted into T3 (triiodothyronine) The thyroid-stimulating hormone (TSH), such a creative name, controls T4 production and is produced by the pituitary gland. When your thyroid is not functioning correctly (hypothyroidism), it can cause sexual dysfunction, depression, increased sensitivity to cold, dry hair and skin, weight gain, and, you guessed it, fatigue.
Sleep apnea occurs when a sleeper either stops breathing momentarily or breathes very shallow breaths. (Mayo Clinic) The end result is lack of restful sleep and daytime fatigue. Sleep apnea can also lead to long-term health complications, such as cardiovascular diseases and high blood pressure. Sleep apnea can easily be relieved with a CPAP (Continuous Pressure Airway Pressure) machine or oral appliances. Sleep apnea may be relieved with weight loss or change in sleeping position.
